The Active Aging Program of Salt Lake County Aging and Adult
Services, is recruiting Spanish speaking volunteers who are interested in
teaching a chronic condition self-management class.
Living Well with Chronic Conditions
This 6 week program teaches self-management skills to people
suffering with a chronic condition or diabetes.
Some of the skills that this program teaches are: goal setting,
problem solving, dealing with emotions, finding resources and support,
communication, healthy eating , exercise, treatment decision, and medication
